Care Coordination and Navigation

Our Social Care Navigators help streamline care and improve the health and well-being of individuals and families. As a contracted provider under the NY-1115 Waiver, FEN delivers Health-Related Social Needs (HRSN) services to the Medicaid population. HRSN services include care management as well as assistance with housing, nutrition, and transportation needs.

Eligibility requirements do apply, including active enrollment in Medicaid and having two or more chronic health conditions such as, asthma, diabetes, heart disease, mental health condition, or substance use disorder. Alternatively, those with a diagnosed serious mental health illness, who are living with HIV/AIDS, or pregnant and post-partum women.
