Care Coordination and Navigation

Care Coordination and Navigation is a NY-1115 Waiver program. Our Social Care Coordinator and Social Care Navigator work with eligible Medicaid Members to address their Health-Related Social Needs, aiming to improve the health and well-being of individuals and families. Our staff connects Medicaid Members who qualify to temporary services such as housing supports, food and nutrition assistance, transportation, and navigation to social care services.
